Kolkata, October 31, 2026: At the 28th National Conference on Pulmonary Diseases (NAPCON 2026), leading pulmonologists, critical care specialists, and thoracic experts gathered to discuss evidence-based interventions for managing complex non-malignant pleural effusions (NMPE) and malignant pleural effusions (MPE). Highlighting the scientific assembly, expert sessions addressed complex clinical dilemmas spanning hepatic hydrothorax, chronic kidney disease volume overload, and end-stage organ dysfunction—conditions marked by high morbidity, frequent hospital readmissions, and elevated mortality rates, particularly when presenting as bilateral transudative effusions.
The panel discussion titled “Recurrent pleural effusions - diagnostic dilemmas and treatment strategies,” moderated by Dr. Subhasis Ghosh alongside panelists Dr. Eihab Bedawi, Dr. Soumyadeep Ghosh, Dr. Haradhan Mandal, Dr. Priyajit Jena, and Dr. Praveen Shahi, experts established that the primary management step for non-malignant effusions is optimizing treatment for the underlying organ failure. Standard protocols mandate optimizing diuretic regimens for heart failure over a 72-hour period or addressing liver dysfunction before initiating invasive pleural drainage. The panel strongly cautioned against performing more than two to three repeated thoracenteses, as frequent aspirations in patients with underlying organ failure lead to severe protein and mineral depletion, systemic immunosuppression, and rapid catabolic decline. Furthermore, while conservative observation is acceptable for mild, diagnosed effusions, failing to drain moderate-to-severe undiagnosed effusions introduces significant risks of missed parapneumonic infections, complex loculations, persistent trapped lung, and irreversible pleural thickening. For patients refractory to medical management, specialized pathways such as Transjugular Intrahepatic Portosystemic Shunt (TIPS) or liver transplant evaluation take precedence over early Indwelling Pleural Catheter (IPC) placement, primarily due to the heightened risk of IPC-related bacterial peritonitis and systemic infection in end-stage liver disease patients.
Complementing the panel discussion, Dr. Rahul Tyagi presented a comparative clinical evaluation on “IPC vs talc pleurodesis for malignant effusion: How to choose?”, analyzing data from clinical trials including the TAP-IT and REDUCE studies to guide therapeutic selection. Data shows that while both IPC and talc chemical pleurodesis provide comparable symptom relief for dyspnea and overall quality of life, IPC placement results in significantly shorter effusion-related hospital stay durations and a reduced need for repeated invasive procedures. However, chemical pleurodesis offers a higher definitive success rate of up to 100% in patients with expandable lungs, whereas spontaneous pleurodesis via IPC occurs in approximately 25.4% of cases. For clinical decision-making, IPC remains the mandatory first-line intervention for non-expandable or trapped lungs where pleural apposition is impossible, whereas talc pleurodesis is preferred for patients with expandable lungs and a longer life expectancy who wish to avoid long-term indwelling hardware. Emerging hybrid approaches, such as outpatient talc administration through an IPC, are increasingly proving effective by achieving successful pleurodesis within 35 days while minimizing inpatient bed days and overall healthcare burden.
The delegates and panelists at NAPCON 2026 agreed that managing recurrent and refractory pleural effusions requires moving away from one-size-fits-all procedures toward highly personalized, symptom-guided algorithms. By balancing underlying organ optimization, patient life expectancy, lung expandability, and quality-of-life priorities, multidisciplinary medical teams can significantly minimize unnecessary hospitalizations and clinical complications for palliative and critical care patients alike.
